This session we will discuss this interactive tool that covers all the components in writing a Transition Plan. Starting with selecting and administering assessment(s), writing measurable goals and supporting actives, development of a quality courses of study, affirming the presence of annual academic goals that support the post school goals and how to make sure agency involvement is present when/if needed.

Principles of STEM integration will be discussed and illustrated through the activities, and we will discuss how domain-specific learning goals might be achieved.
